Parent/Student iPad Agreement Form

iPad Parent Form for Grades 5-12

Parents/guardians of 5th-12th graders must complete the following paperwork and turn it in along with the annual $20 technology fee/student before an iPad can be issued. The paperwork must be turned in by a parent/guardian to your child's school. Pay your technology fee online @ www.myschoolbucks.com via debit/credit card or e-check!
